Journey of a Braveheart Kid on Multiple ECMO for Post-Viral Pneumonia

SKMC’s Multidisciplinary Team Successfully Manages Complex Case, Leading to a Remarkable Recovery

 

Sheikh Khalifa Medical City (SKMC) is proud to share the inspiring story of a 4-year-old child who overcame severe respiratory failure due to viral pneumonia, thanks to the dedication and expertise of our multidisciplinary team.

 

The young patient was referred to SKMC with severe respiratory failure secondary to viral pneumonia, which led to bilateral pneumothorax post-intubation and ventilation. The situation was critical, requiring immediate and advanced medical intervention.

 

Our team established multisite VV ECMO with 19Fr RIJV access and 19Fr FV return. Despite numerous challenges, including a circuit change on the 15th day due to clot burden and a sudden cardiovascular collapse on the 31st day, the team remained resolute. The patient underwent multiple ECMO conversions and circuit changes, along with significant surgical interventions, including Broncho Pleural Fistula repair and resection of the left lower lobe of the lung.

 

After nearly four months in the intensive care unit, the patient showed remarkable improvement. High Frequency Oscillation (HFO) settings and prone ventilation were implemented for lung recruitment, and repeated bronchoscopies were performed to remove clots. The patient successfully tolerated a trial off ECMO and was decannulated, leading to a stable discharge.
